Below you will find our Square checkout portal. Please input the amount from your most recent invoice in the box that is labeled "Enter Amount." Press Checkout and you will be taken to a secure website where you can input your credit or debit card information.

Private Pay

Occasionally clients do not want to use their insurance and prefer to pay privately for services. Private pay clients are billed $250 for an intake session and $175.00 per 55 minute session.

A sliding fee scale may be available for clients who are uninsured or under-insured. Please contact the office for additional information.
Insurance Coverage

We are in-network with most major insurance providers in Minnesota. See accepted insurances.

While we are able to verify coverage with most insurance companies, the estimation of insurance coverage is not a guarantee of coverage.

Late Cancellation/No-Show

No-shows and cancellations with less than 24 hour advance notice may incur a $125.00 fee. The late cancellation/no show fee is not covered by insurance and is the client’s sole responsibility. This policy is designed to protect our clinicians’ time and not to penalize our clients financially.

Clients with commercial insurance will be billed automatically for late cancel or no-show appointments. After two no-show or late cancelled appointments clients may be referred elsewhere for services. View Full Late Cancellation/No-Show Policy here.
Employee Assistance Programs (EAPs)

We accept EAPs through OPTUM insurance.

If you would like to use your EAP benefit, we require a copy of the authorization letter that includes your EAP code, number of sessions approved, and your social security number. We cannot bill the EAP without this information. Even when using an EAP we require primary insurance information.

If you call our office after hours for an emergency, you will be directed to call 988 or to go to your local emergency room. LMHC is not a crisis clinic and our clinicians are not available for crisis calls.
