Fee schedule for record review; IME interviews; collateral contacts, either face-to-face or via telephone or email;

court-related correspondence; report writing; deposition and court appearances:

 

IME-related record reviews; interviews; collateral contacts, either face-to-face or via telephone or email; report writing; and court-related correspondence are billed at $350.00 per hour, or fraction thereof.  A $900.00 fee is charged for IME interviews that are canceled less than five working days from their scheduled date.  A minimum $2,500.00 retainer is required at least five business days prior to the scheduled IME. Larger retainers maybe required and are set on an individual case basis.

 

Court fees for deposition or court appearances, and travel are billed at a half day minimum charge of $450.00 per hour. An $1,800.00 retainer is required five working days prior to Dr. Mullenbach’s scheduled appearance. The retainer will not be returned for any deposition or court appearance that is canceled less than five working days from Dr. Mullenbach’s scheduled appearance.
